Findlay St., Carey. Appointments are required and may be made by calling WCPH at 419-294-3852. The child’s health insurance information should be available when scheduling the appointment. Back-to-school vaccination is a crucial step in ensuring Wyandot County students have a safe and healthful year. Different vaccinations are required based upon the child’s age. By the first day of kindergarten, students should have: — Five doses of DTaP vaccine. — Four doses Polio vaccine. — Two doses MMR vaccine. — Three doses Hepatitis B vaccine. — Two doses Varicella (chickenpox) vaccine. — A current physical on file. Seventh grade students should have: — One dose of the Tdap vaccine. — One dose of the Meningitis (ACWY) vaccine. By the first day of 12th grade, students should have: — One dose of meningitis (ACWY) vaccine. Students are eligible for this dose after age 16.
